Chronicle Diabetes (CD) is provided free to our Recognized programs to document their DSMES services. Chronicle is designed to meet the requirements of the Health Insurance Portability and Accountability Act (HIPAA) and satisfies all of the ADA requirements for documenting participant information. The only time a service will have a fee involved with CD use is if a program has a special request or need that the ADA staff cannot perform.  Chronicle Diabetes is a web based standalone platform that is currently not able to integrate with existing Electronic Health Records. 

Please Note:  Chronicle Diabetes is provided for free to ADA Recognized Services. There will be a cost involved if your service requires any security documents to be completed prior to using the platform. 

We are excited to announce that Chronicle Pro is now live.  If you were not able to attend the Chronicle Pro webinar, we have the recording of the presentation below.  

Services will be able to utilize Chronicle Pro free for 90 days starting October 16th.   After 90 days the Chronicle Pro features will be disabled, and your service can either subscribe to Chronicle Pro or continue to use the current version of Chronicle. 

Chronicle Pro Subscription Fees:

  • 1 Year $150.00 
  • 4 Year $400.00 ($200 savings)

Please Note: The current version of Chronicle Diabetes will remain free to all Recognized Services.

Chronicle Pro Features:

  • Each section of the participant chart will have an A-I indicator letting the user know if that component of the cycle has been met (red light-no / green light-yes).
  • Two new sections have been added: One section for documenting the referral information and another to document the provider communication (A & I within the cycle).  Both sections will have an upload function to house any documentation. 
  • Users now can filter participants based on if they have met all the A-I requirements within the participants search tool.
  • A new patient audit report will be added to allow the user to quickly print a complete chart that has met the A-I requirements.
  • Calendar function: A new calendar function will allow users to see upcoming classes, sessions, and scheduled follow-ups.

The ERP and Chronicle development team have been hard at work to launch the Chronicle refresh!  The goal for this refresh is to optimize the platform to support current browsers as well as streamline a few of the tedious data entry points i.e., Participant Assessment and Health Status sections.   This refresh will not change how you document DSMES information, and it won’t affect any of your historical information.

Chronicle Diabetes is now available to non-ADA Recognized services for a nominal subscription fee:

  • $100 on-boarding fee
  • $250 start-up and subsequent annual fee

To begin, please submit the ERP Onboarding Form and choose the “Non-ADA Recognized Chronicle Subscription” option.   Once this is submitted the Quality Coordinator will receive a welcome email with instructions on getting started and submitting the subscription within the portal.

Please Note: Business Associate Addendum (BAA) for Chronicle Users (The BAA must be accepted within the ERP Portal by the Quality Coordinator prior to accessing Chronicle. We recommend sending this document to your Legal Counsel for review.)

Already Documenting in an Electronic Health Record?

Chronicle can be used at minimal capacity which will provide you with the ability to export individual and aggerate outcomes (Behavioral Change Objectives & Participant Outcomes) as well as import data to ERP applications and annual status reports (ASRs).  For more information about Chronicle reports, please see below. 

The Chronicle Minimal Capacity Guide will provide you with the 3-step process in entering data at minimal capacity as well as an optional 4th step to indicate the education type. 

We are excited to share with you that you can now access the Chronicle demo site below.   This is a demo site so please do not enter any real patient health information.  Each day you would like to access the site you will need to request a new temporary username and password. Once you've completed this information, a temporary username and password will be emailed to you. This information will be valid for the remainder of the day, after which time it will expire. If you would like access to the demo on a later day, you will need to request a new username and password.

Getting Started with Chronicle: 

We are excited to share with you that the Chronicle High-Level report is now available within the Chronicle Diabetes reports tab and the ERP portal. This report can be downloaded as a standalone report, or it can be exported through the filter report which allows you to filter report data based on provider, demographics, clinical lab information, and numerous other filters chosen by the educator. This report can also be downloaded program-wide to include all of your recognized sites. Download the Chronicle High-Level Program report guide for more information and instructions.

Chronicle Diabetes (CD) FAQ

Q: Does CD integrate with other electronic health record (EHR)?

A: CD is a standalone documentation platform that does not currently integrate with other EHRs.
 

Q: Is CD HIPAA compliant?

A: CD is designed to meet the requirements of the Health Insurance Portability and Accountability Act (HIPAA)
 

Q: Is there any software I need to download to use CD?

A: CD is a web-based platform so there is no software to download.  CD can be accessed from any computer or mobile device as-long-as there is internet access.  We recommend using the most recent version of your browser when accessing CD.  Chrome and Firefox are the recommended browsers.
 

Q: Is there a cost associated with using CD?

A: CD is a free benefit to Recognized services and services that plan on applying for Recognition in the future.  The only time a service will have a fee involved with CD use is if a service has a special request or need that the ADA staff cannot perform.
 

Q: Does the ADA require services to use CD in order to maintain Recognition?

A: ADA does not require services to use CD for documentation purposes.  CD is a free benefit of Recognition for services to use if they choose to. 


Q: If I am already documenting in an EHR do I need to double document in CD?

A: If your current EHR documents everything you need for your DSMES service and Recognition, then there is no need to double document in CD.  Services often use CD at minimal capacity if their current EHR does not aggregate their behavioral or participant outcomes.  Learn more about CD minimal capacity use here


Q: My service is planning to apply for Recognition in future.  Can I use CD before submitting an application?

A: Yes.  Services that plan on applying for Recognition in the future can use CD prior to submitting an original application.  A service will need to submit an onboarding form to be setup within the ERP Portal in order to be able to access CD. 


Q: What are the benefits of using CD?

A: CD is built to satisfy all ADA requirements for documenting participant information.

  • Import data required for ERP Applications and Annual Status Reports
  • Document and track the complete patient DSME Cycle
  • Track educator hours and patient statistics
  • Generate Letters to patients and providers
  • Generate reports for the purpose of advertising program outcomes and use for QI projects
