Overview

The Guidelines in Action Showcase invites health care professionals to share real-world examples of how they’ve implemented the latest Standards of Care in daily practice. We’re looking for innovative, practical, and impactful solutions that demonstrate measurable improvements in diabetes care—whether through enhanced workflows, technology integration, patient-centered initiatives, or health system improvements.

This is your opportunity to share your success, inspire peers across the field during the 2026 Clinical Update Conference (CUC), and highlight the transformative impact of applying the Standards of Care in real-world settings.

What We're Looking For

Submissions should demonstrate how you’ve:

  • Integrated the 2025 Standards of Care recommendations into clinical practice or systems of care
  • Improved patient outcomes or care experiences
  • Developed tools, workflows, or quality improvement projects aligned with the 2025 Standards of Care
  • Leveraged technology or multidisciplinary collaboration to advance care

 

Each submission (300–500 words) should include:

  • Title
  • Background
  • Methods
  • Results/outcomes
  • Conclusions/lessons learned

Optional focus tracks:

  • Diabetes management
  • Patient education/self-management
  • Case management
  • Health system implementation
  • Access to care/population health
  • Obesity management

A $100 application fee will be collected during the submission process.

Timeline

  • Call for submissions: November 5–28, 2025
  • Winners announced: January 5, 2026
  • Posters on display at the CUC: February 6, 2026

Recognition and Awards

The top 12 submissions will be selected as Guidelines in Action Showcase winners.

Winners will have their posters displayed on-site at the CUC ’26 and participate in the Welcome & Networking Reception taking place on February 6 to share their work in person.
