Standards of Medical Care in Diabetes The American Diabetes Association publishes the Standards of Medical Care in Diabetes annually. These rigorously reviewed recommendations provide clinicians with guidance to prevent and treat diabetes and its complications. In addition to the full Standards of Care, an abridged version edited by primary care providers for primary care providers, is available. 

National Standards for Diabetes Self-Management Education and Support. Diabetes self-management education and support (DSME/S) provides the foundation to help people with diabetes to navigate these decisions and activities and has been shown to improve health outcomes.

  • Chronicle Diabetes (ERP Only) Chronicle Diabetes (CD) is provided  free to our Recognized programs. The only time a program will have a fee involved with CD use is if a program has a special request or need that the ADA staff cannot perform.
