In order to submit an application, you must log in to your ADA account (my.diabetes.org).
For Returning users: Your username for logging in is your primary email address that is on file with ADA. To activate your account or retrieve your password, use the "Forgot your password?" link.
Then, begin an award application by selecting “Award Applications” on the left navigation bar.

To resubmit a previous year's application, start a new Award application. On the third screen, select the option "I want to resubmit a nomination from a previous year." You'll then be able to choose from a list of past submissions.

You will see that your application was successfully submitted by clicking “My Awards” on the left navigation. You should also receive a confirmation email from the ADA shortly after you've completed your submission.
Yes, the nominee must be a current member in order to be eligible. The nominator, however, does not to be a member to nominate BUT does need to have a Diabetes Pro Account / Profile.
