* If you did not pre-register or are receiving a log-in failure message, please email professionaleducation@diabetes.org. Be sure to provide all of your contact information including your first and last name, email address, which type of credit you wish to receive (i.e. MD, FP,DPM, RN, NP, RPH, RD, PSYCH, other), and mailing address. You will then be granted access to the online evaluation within 24 hours.
