Results: The objectives of the study was to see the rheumatological manifestations and clinical presentation of rheumatological syndromes and diseases in diabetes mellitus(DM).
75 diabetic cases admitted in various wards of medicine department were taken up for the study.All the patients were thoroughly examined, clinically and various musculoskeletal manifestations noted. Diabetes was diagnosed on the basis of WHO criteria for diagnosis of diabetes mellitus. Patients taken were more than 12 years of age and of both sexes. Blood sugar was estimated by glucose oxidase and peroxidase method. The patients having sepsis, acute hepatitis, pregnancy, other rheumatological disorders , other autoimmune diseases were excluded from the study.
Out of 75 cases, 70(93.33%) were Type 2 DM and 5(6.67%) were of Type 1.Male patients were 42(56%) and female 33(44%). Out of 75 cases 54 had rheumatological disorders, 49 cases of Type 2 DM and 5 of Type 1 DM. Out of the 54 patients affected 28 were male and 26 female.Diabetic hand syndrome (DHS-LJM)(10) was the most common finding followed by adhesive capsulitis (ADC)(8), hyperostosis (7), calcium pyrophosphate dehydrogenase disease (6), shoulder hand syndrome (4), flexor tenosynovitis (4), Dupuytren's contracture (3), carpal tunnel syndrome (3), gout and hyperuricemia (3), skin manifestation (2), and 1 case each of osteolysis, osteopenia, osteoarthritis, neuroarthropathy. Among the 54 patients 24(44.44%) had high glycosylated haemoglobin (HbA1c) levels (8.5-10) . Duration of the illness was greater than 5 years in 29(53.7%) patients and greater than 10 years in 7(12.97%) patients. Rheumatological manifestations were maximum in the late age group (51-60 yrs). LJM and gout with hyperuricemia was related to T1DM.
From the study it can be concluded that rheumatological manifestation are common association and complication of diabetes mellitus. It is directly associated with duration of the disease. Most common is DHS-LJM followed by ADC. High HbA1c value directly correlates with rheumatological manifestations and severity of the illness.
